Activity Description

This recorded webinar featured a panel of subject matter experts, explores the unique challenges and opportunities to support adolescent and young adult (AYA) survivors of cancer. The role of primary care clinicians in managing long-term follow up care, strategies to strengthen communication between oncology and primary care teams, and resources to support AYA survivors, their caregivers, and healthcare professionals will be discussed, with opportunities for input from the audience.

Target Audience

This activity is intended for hematologists/oncologists, advanced practice providers, nurses, psychologists, social workers, and other healthcare professionals involved in the care of adolescent and young adult (AYA) survivors of cancer.

Learning Objectives

    • Describe the unique survivorship challenges AYA patients face during care transitions and integration with primary care
    • Explain the important role of primary care clinicians in delivering long-term follow up care for AYA cancer survivors
